Community Case Management
Human Service/ General Assistance
CHS currently manages the General Assistance programs for the Towns of Derry, Hampstead, Londonderry and Plaistow. Residents in need of emergency aid.
Behavorial Health
A level one treatment service which provides patients of the practice who need care with short-term, goal oriented counseling and medication management
South Central Public Health Network
A collaborative of partner agencies that are working to enhance and improve community health and public health services across the region.
If you are struggling to navigate the complicated system of service access and need help, CHS will:
- Be ready to assist with information and support including, Case Management to help you identify support and community resources for which you might be eligible and to increase knowledge and skills geared towards personal and financial independence
- In addition, CHS manages the General Assistance Programs residents of Derry, Hampstead, Londonderry and Plaistow.
While appointments are recommended it is almost always possible to speak with a member of the staff prior to coming in to discuss your individual circumstance prior to a meeting.
