CHS has provided services and support to vulnerable residents of the Greater Derry area since 1989. Funded through grants, donations and some contracts, CHS works every day to support individuals and families to identify the resources the supports that they need while supporting them to increase their knowledge and skills with an eye towards self-sufficiency and well-being.
Community Case Management
Exploring options for the help that you might need when faced with stress, financial difficulty and limited knowledge of the human services system can be frightening.
CHS has a professional Case Management staff that is available to meet with any resident of its service area at no charge to explore both public and private community resource connections for which you may be eligible.
Human Service/General Assistance
CHS currently manages the General Assistance programs for the Towns of Derry, Hampstead, Londonderry and Plaistow.
Residents in need of emergency aid which includes but is not limited to rent, fuel or electric assistance should call 603-425-2545 and speak with a member of the case management team prior to picking up an application.
Behavioral Health
Working in partnership with the DMC Primary Care, CHS manages the Behavioral Health Department for the Practice.
The Behavioral Health Department is a level one treatment service which provides patients of the practice who need care with short-term, goal oriented counseling and medication management if needed while coping with depression, anxiety and a range of other issues including the stressors of living with a chronic illness or new diagnosis.
South Central Public Health Network
The South Central NH Public Health Network (South Central PHN) is a collaborative of partner agencies that are working to enhance and improve community health and public health services across the region.
